Pavillion is our state-of-the-art sub-acute rehabilitation unit.
The Pavilion is ideal for those requiring nursing care on a short-term basis. This 10-bed unit is designed for those recovering from injury, stroke, general and cardiac surgery, hip and joint replacement, wound care, intravenous therapy and generalized weakness from a prolonged hospital stay.
Our experienced rehabilitation staff works to optimize the return of functional abilities and to maximize independence during recovery. A customized care plan is implemented and designed to meet each resident’s specific needs, offering the opportunity to return home or move into our assisted living residence. Many amenities complement care and recovery.
Sub-Acute Statistics
- 100% percentage of residents whose functional abilities were assessed & functional goals were included in the treatment plan the national Average is 99.1.
- 97.5% percentage of residents whose medications were reviewed & who received follow up care when medication issues were identified the national average is 88.4%.
- 100% short stay residents who needed & got flu shot for the current flu season the national average is 80% and New Jersey is 87.1%.
- 100% of short stay residents who needed 7 got a vaccine to prevent pneumonia the national average is 80.3% and New Jersey is 86%.
- 5.3% percentage of short stay residents who require emergency room visit the national average is 9.7% and New Jersey 7.5%.
- 61.6% percentage rate of successful return to home & community from a skilled nursing facility the national average is 52.9%.
